ADHS Requirements for Establishing a Service Site
The Arizona Department of Health Services (ADHS) FY-2015 Conrad Waiver Program will likely launch in October 2014. ADHS has traditionally required that a service site seeking to place a J-1 waiver physician be operational for at least six months prior to making an application for a J-1 waiver. It also requires documentation of recruitment efforts for the particular service site in the six months prior to application. If you plan to hire a J-1 physician to work in an underserved area next summer, please ensure that the service site is created and operational by April 1, 2014 and recruitment efforts are started or continued.
